1. WHAT INFORMATION DO WE COLLECT?

  

In Short: We collect only personal information that you provide to us.

  

We collect personal information that you voluntarily provide to us when you request an appointment for services.  HOWEVER, WE WILL PROVIDE CONSULATATIONS WITH ANONYMITY AND COMPLETE PRIVACY AND CONFIDENTIALITY - YOU ARE NOT OBLIGED TO PROVIDE ANY PERSONAL INFORMATION BEFORE OR DURING A CLINICAL CONSULTATION.

  

When you request a formal evaluation and/or individual psychotherapy, we may ask for information, including but not limited to, the following:  Your name, a contact phone number, a contact email address, a mailing address, and your contact preferences.  However, some or all of this information may not be necessary in order to engage in services.  This can be discussed during the initial consultation.

  

Sensitive Information. We do not process sensitive information.

   

2. HOW DO WE PROCESS AND/OR STORE YOUR INFORMATION?

  

Voluntarily submitted digital information is stored as an encrypted digital file(s).  However, upon your request, we will use solely hand-written, hard-copy notes that are double-locked (i.e., 2 layers of physical locks in a single location) in our secured office.

  

3. WHEN AND WITH WHOM DO WE SHARE YOUR PERSONAL INFORMATION?

  

We do NOT share your information with third parties.

  

4. HOW LONG DO WE KEEP YOUR INFORMATION?

  

In Short: We keep your information for as long as necessary to fulfill the purposes outlined in this privacy notice unless otherwise required by law.

    

We will only keep your personal information for as long as it is necessary for the purposes set out in this privacy notice, unless a longer retention period is required or permitted by law (such as tax, accounting, or other legal requirements).

  

When we have no ongoing legitimate business need to process your personal information, we will either delete or anonymize such information, or, if this is not possible (for example, because your personal information has been stored in backup archives), then we will securely store your personal information and isolate it from any further processing until deletion is possible.

  

Information in hard-copy form is physically destroyed via crypto-cut shredding and incineration.

 

5. HOW DO WE KEEP YOUR INFORMATION SAFE?

  

In Short: We aim to protect your personal information through a system of organizational and technical security measures.

  

We have implemented appropriate and reasonable technical and organizational security measures designed to protect the security of any personal information we process. However, despite our safeguards and efforts to secure your information, no electronic transmission over the Internet or information storage technology can be guaranteed to be 100% secure, so we cannot promise or guarantee that hackers, cybercriminals, or other unauthorized third parties will not be able to defeat our security and improperly collect access steal or modify your information.  Although we will do our best to protect your personal information, transmission of personal information to and from our website is at your own risk. You should only access the website within a secure environment.

  

Physical (e.g., “hard-copy”) information is secured in a double-locked location, in accordance with APA ethical standards.  A “double-locked” location is a physical space where physical documents have been secured by tangible means (e.g., padlocks, deadbolts, chain locks, combination locks, etc.) twice-over.  That is, the documents are secured in a locked (as defined above) entity that is within a locked (as defined above) room within a building or edifice.

  

6. WHAT ARE YOUR PRIVACY RIGHTS?

  

In Short: You may review, change, or request the destruction of any information you have submitted at any time.

  

You can ask for the deletion/destruction of any of the personal information that you may have shared with us. If you ask us to delete or destroy your personal information, we will respect your request and delete your personal information, subject to certain exceptions provided by law, such as (but not limited to) the exercise by another consumer of his or her right to free speech, our compliance requirements resulting from a legal obligation, or any processing that may be required to protect against illegal activities.
